False PHD is a passive item introduced in The Binding of Isaac: Repentance expansion. Found primarily in Devil Rooms and Curse Rooms, it acts as the malevolent counterpart to the standard PHD item. Instead of making all pills positive, it leans into negative effects while granting powerful offensive and defensive compensation. Core Effects
Pill Identification: It instantly identifies the effects of all pills upon pickup, ensuring you always know what you are taking.
Immediate Health: Spawns one Black Heart on the floor when you first pick up the item.
Pill Inverse Conversion: Converts positive stat-modifying pills into their negative counterparts (e.g., Tears Up becomes Tears Down).
Stat-Down Damage Buff: Consuming any stat-decreasing pill rewards you with a permanent +0.6 Damage Up alongside the stat penalty.
Non-Stat Pill Defense: Consuming any bad or neutral pill that does not decrease a stat spawns a Black Heart on the ground. Top Synergies
Rock Bottom: This is the ultimate synergy. It prevents your stats from actually decreasing, allowing you to freely chug stat-down pills for pure, unpenalized damage upgrades.
The Wafer + Placebo: If you use a Bad Trip pill, The Wafer limits the damage to half a heart, while False PHD spawns a full Black Heart—allowing you to farm infinite health.
A Pony / White Pony: These items lock your movement speed to a minimum of 1.5, effectively nullifying the downside of Speed Down pills while you reap the damage boosts.
PHD / Lucky Foot / Virgo: If you hold False PHD alongside any of these “good pill” items, they cancel out the forced negative conversion. Pills can appear as both good or bad again, but taking any bad pill will still trigger the False PHD damage or Black Heart bonuses. Strategy and Value
